In order to arrange a meeting with one of our staff, please call the office during business hours, between 9 am and 5 pm, Monday through Friday, at (724) 282-1627.  When you call, here are some of the questions our staff will ask you:

• your name, age, address, and other identifying information

• a description of the problem for which you are seeking help

• a brief review of your medical history, including your current medications

• insurance information

• the name of an emergency contact person

Except in the case of children and the very elderly, we ask that you make the initial call for an appointment for yourself, rather than to have a family member do it for you.  However, if you are seeking treatment for several family members, any family member can make the initial call.
